Interim Guidance for Families – Until a national system is transparently established, (Before We Die) BWD advises families to formally document their needs by:
1. Applying to the Local Authority for the Social Housing List (include a HMD1 disability form link).
The form will be specific to your local authority (council).
Just go to their website and type Social Housing Application Form into the search bar.

2. Requesting a Disability Support Application Management Tool (DSMAT) assessment from your service provider and ensuring it is submitted to the HSE.
3. Writing to your HSE Area Disability Manager to formally request a residential placement. You will find their address on the HSE website.
4. Ask the HSE disability section for a case manager for your adult with disabilities, as this links your loved one to the HSE. They will do their assessment, and you are now on the HSE “books”.
